Soon your child will start childcare at SKO. In order to be able to take care of your child, we would like to receive more information about him/her. Therefore we request that you fill out this form.
Before your child starts childcare at SKO, you will have an introduction meeting with his/her mentor. During this meeting you will discuss your answers and have the opportunity to give more information.
While filling in the form, you can skipp the questions you would like to discuss further. The mentor will complete the form based on the information you provide during the meeting. After the meeting you will receice a copy of this form by email (including the additions made by the mentor).
We recommend that you fill out this form on a computer or tablet and complete it without interruptions, because answers are not saved untill you send in the form.
